What the Study Found
Children’s willingness to try new foods jumped from a baseline of 8–15% to 76–91% after 28 sessions over ten weeks, according to a working paper posted on SSRN by Sofya A. H. Bajaa and colleagues at Constructor University in Bremen, Germany. Food-related behavioral issues dropped by 41–67%.
The sample was small: three children with ASD, aged two to six, studied in a single-case experimental design replicated across three families. Bajaa, a PhD candidate in Constructor University’s Department of Psychology and Methods, developed the Schmetterling Nutritional Behavior Intervention (NBI) program as part of her doctoral research, with senior co-author Ahmed A. Karim of the University of Tübingen.
The program trains parents as co-therapists. Sessions use positive reinforcement, gradual food exposure, imitation chaining, and shaping, all established behavioral techniques, combined with what the authors call therapist-guided “exoskeleton modeling,” a technique in which the therapist physically guides the child’s motor movements during feeding. The design is meant to let parents replicate the methods at home after the structured sessions end.

A Second Study, Same Team
A separate retrospective study, posted as a preprint on Research Square in June 2026, compared children who completed the Schmetterling NBI program with a standard-care control group. The authors reported significant reductions in selective eating, measured by the Children’s Eating Behaviour Questionnaire (CEBQ), lower autism symptom severity on the CARS-2 assessment, and improved autonomic regulation, including increased heart rate variability and a lower stress index.
Neither study has been published in a peer-reviewed journal. The only peer-reviewed Schmetterling paper to date, published in the International Journal of Developmental Science (SAGE, 2026), covers the Motor Treatment arm of the program, which addresses motor skills, not feeding. No independent research group has replicated the NBI feeding results.
Why It Matters for ABA Practice
Food selectivity affects an estimated 63% of children with ASD. The results were published in the peer-reviewed Journal of Eating Disorders in May 2026, according to a 2025 meta-analysis in Annales Médico-Psychologiques that pooled data from 1,458 individuals across six studies. Feeding therapy already falls within the BCBA scope of practice; ABA-based approaches typically use stimulus fading, differential reinforcement, and escape-extinction protocols. Parent and caregiver training for feeding is billable under CPT 97156 (Current Procedural Terminology, the medical billing code system).
The Schmetterling NBI program integrates established behavioral strategies with the authors’ novel components. Whether those additions improve on existing ABA feeding protocols is a question the current evidence, drawn from a three-child sample and two preprints by the same research team, cannot yet answer.
The SSRN working paper is available for review. Peer-reviewed publication of the NBI feeding results, if accepted, would be the next step toward clinical adoption.
